Dr. Burick’s initial consultation is approximately one hour. Patients may call to set up at appointment at 730-9000. At the time of booking the appointment, a credit card will be taken to reserve your appointment time. Please call at least 24 hours in advance of your appointment if you are unable to make it. If a patient does not show or cancel the appointment within 24 hours, a $50 fee will be charged to your credit card. The fee for the initial consultation is $250 and should be paid at the time of the service. Dr. Burick does not participate with any insurances including medicare. A receipt for the visit will be provided for you on a standard insurance form which may be submitted to your insurance for reimbursement. Dr. Burick does not guarantee any specific level of reimbursement. Subsequent follow up visits are scheduled to last 30 minutes. A fee of $125 must be paid at the time of the service. Phone consultations may be scheduled for a patient unable to return to the office for follow up appointments due to travel, distance or time restrictions. If you wish to schedule a half hour phone consultation to review signs or symptoms and lab data, prescription changes, etc. you may arrange this with our receptionist. Your credit card information will be taken prior to the consultation and charged following your consultation. Patients will not be charged for email communication or phone calls lasting less than 15 minutes. Saliva test kits are available at the Burick Center and the patient will be charged upon receipt. The saliva test kit instructions will be reviewed by our nurse. The cost of a saliva test kit is $150. Insurance does not cover the cost of these kits. Laboratory tests are not drawn or performed at the Burick Center. A prescription for blood work may be provided at the time of your visit with specific instructions. The patient is responsible for choosing a lab with whom their insurance participates. An additional option for patients is to become a member of the Burick Center for Health and Wellness. Your initial consultation fee may be applied to your first annual membership if you decide to join within 30 days of your initial consultation.
