You’re invited to become a charter member of this exciting new concierge medical practice. The practice’s enrollment is extremely limited and gives you access to the very best in healthcare, in a warm and healing environment. Benefits include the following:

• Dr. Burick will serve as your primary care physician and provide you with diagnosis and management of acute and complex medical problems.

• Referrals and scheduled appointments for ancillary services such as radiology, out patient testing, etc. as well as referrals to non-traditional services (such as nutrition, acupuncture and massage therapy).

• Four acute care visits per year (with same or next-day appointments).
• Four routine visits per year, including an annual physical with routine diagnostics.

• Assistance facilitating medical information when you travel.
• E-mail communications with Dr. Burick regarding medical care and test results.

In order to maintain this high level of exceptional care and ensure the ability to spend ample time with each patient, practice enrollment is extremely limited and available on a first-come, first-served basis. The practice does not participate in medical insurance or process medical claims. However, a receipt will be generated for each visit for members to submit to their private insurance for personal reimbursement to each client. (The annual retainer, may also be applied toward your insurance deductible.) Tests, such as labs and other ancillary services, may be covered by your health insurance, based on individual coverage.
